Certified Lymphedema Services

Specialty work such as post surgical cosmetic mld and body sculpting (manual not machine) will include product cost added to the session.

PACKAGES

• Withholding medical information voids package.

• Inappropriate behavior voids package.

• I reserve the right to void a package for any reason.

• Appointment cancelled without a 24-hour notice will be counted as a full session in a package.

• No refunds on packages.

• You must come in and get a Gift Certificate if you would like to use one of your package sessions as a gift.

• If you are ill, please don’t come into the office to use your package.

• The package expires one year after purchase.

• Card must be presented if you received one; otherwise, you must pay the regular price for your session and it will not be included in the package.
