• Appointments not cancelled or rescheduled at least 24 hours before your appointment will be subject to fee for service.

• All packages are nonrefundable.

• please allow 72 hours of any vaccination.

• During times of illness, please call ahead to reschedule the appointment. You cannot be seen until the following conditions have cleared:

  if you are currently on antibiotics, have an active infection of ANY kind, including a tooth infection especially.

  Any type of skin rash such as poison oak, poison sumac, a spider bite or a questionable tick bite.

  Flu, Covid, Sinus infection, fever, sore throat, general feeling of being unwell.

• Please note that if you are under a Doctors care you will need a note to be seen. The note should be from the attending Doctor only. I require the complete intake form to be filled out. This includes conditions, illnesses, medications, surgeries. Please be prepared to fill out the form or I will be unable to work with you that day .

• When we speak or text to schedule your appointment please discuss all health conditions so we may move forward in your best interest.
